  • Notification of submission will show on screen and a copy will be sent to the email you provided.
  • The form will not be saved if you navigate away from the form. You will have to start over.
  • Illinois Department of Natural Resources will respond to the provided email address.
  • If you cannot fill out the form, a copy can be downloaded below and emailed to DNR.EEOADA@illinois.gov.
  • A printed version can be filled out by hand and mailed to IDNR at:

  • Permit is not considered granted until a response is received from the Illinois Department of Natural Resources, with any listed adjustments to the request.
