Forms Lists Logs

  • 02/20/2024: The annual active permitted Class II injection well lists were mailed on 02/15-16/2024. This mailing reminds injection well operators of the submittal of Form OG-18 by May 1, 2024. Please allow time for the arrival of your list due to the federal holiday on 02/19/2024. If you don't received your list by 02/29/2024, please contact Hernando Albarracin.
  • 02/06/2024: Form OG-10 and the OG-10 Guidance Document have been revised. Please note that item #37 on the OG-10 form contains clarification about the permit application fees as follows:
    • The nonrefundable $400.00 appliaction fee applies to Oil Production, Water Supply, Gas Production, Coal Bed Gas, and Coal Mine Gas wells.
    • The nonrefundable $300.00 application fee still applies to Gas Storage, Observation, and Service wells.

The Department will amend its adminsitrative rules to rectify the discrepancy. After the rules are amended, the permit application fee to drill, deepen, or convert a production well will be $400.00 for all wells, which was the original intent of the amendments that became effective on Janaury 1, 2023.

  • 01/03/2024: Due to Mark Merritt's retirement from IDNR, questions about the Plugging and Restoration Fund program should be directed to Hernando Albarracin or Jessica Donnelly.
  • 12/26/2023: The OG-15C Annual Temporary Abandonment Certification Form has been revised. First, the title of the form was changed to coincide with the intent of the form. That is, the annual certification period is on a fiscal year basis, or from July 1 of the previous year to June 30 of the current year. Secondly, the question (#3) about the TA fee was removed because the Department will send a TA statement for the TA fee to all operators annually; therefore, the question is not necessary. The revised form is available below in "Oil and Gas Forms."
  • 12/13/2023: The OG-15 Requet for Temporary Abandonment form has been revised. A place to print the name of the person authorized to sign the form was added. Also, the notes at the bottom of form were updated. Effective immediately, please use the December 2023 version of the form, which is available below in "Oil and Gas Forms."
  • 10/23/2023: On January 1, 2023, the  amendments of the Temporary Abandonment (TA) regulations became effective.  These regulations are codified at 62 Illinois Administrative Code 240.1130. Please read more information in the Frequently Asked Questions (FAQs). There are two important changes regarding the application for TA status. First, you must mail initial requests (see FAQs for the definition of initial request) to the Springfield office. Second, you must use the revised OG-15 form, which is available below in "Oil and Gas Forms."
  • 10/03/2023: Form OG-01 has been revised. Now it contains a fifth choice for question 1. Please use "Other" if your business structure is a Trust, an Estate, or other type.
  • 08/22/2023: All the Notices of Annual Well Fee for FY2024 and the Notices of Temporary Abandonment (TA) Fee for FY2024 have been mailed out to the operators. If you did not receive yours, please contact Hernando Albarracin. Please remember that annual well fees increased effective January 1, 2023, and the new TA fees went into effect also on January 1, 2023. See the table below for more information.
  • 05/01/2023: Questions about requests for transfer of permits should be directed to Kyle Fitzpatrick or Jennifer Clarke.
  • 02/15/2023: Beginning January 1, 2023, permit application fees and annual well fees have increased as indicated below. Please note that the fee for Application for a Liquid Oilfield Waste Transportation Vehicle Tank Permit has also increased from $100.00 to $150.00. That was inadvertently omitted from the prior announcement on January 1, 2023. In addition, the Application for a Liquid Oilfield Waste Transportation Vehicle Tank Permit form (OG-16A) has also been updated to reflect the fee increase. (See "Oil and Gas Forms" below.)
