Safety Education Card Search
To request an Illinois safety education certification card replacement, click on the button below that corresponds with how you completed the class. Your information must be entered exactly as it appeared on your original certification card (maiden name, nick-name, etc.).
If you completed the Illinois safety education class in-person, you can print a replacement card online for free. If you completed the class online, the online course vendor will mail you a new card for a fee of $12.99 and will provide you a temporary replacement card that you can print.
